The registration desk will be located in the Congress Center, University of Warmia and Mazury, Dybowskiego St. 11 (corner of Dybowskiego and Oczapowskiego St.) Olsztyn, Poland.

Those who wish to attend the Congress are requested to fill in the Registration and Accommodation Form and return it to the Organising Committee.

Please, register by e-mail or fax and complete attached Registration Form in BLOCK CAPITALS. Return it to the Congress Secretariat. Please submit one Registration Form for each active delegate.

Please note that only properly completed Registration Form with all required details will be accepted. Remember to make a photocopy of this form for your own records.

Each delegate will receive a confirmation and a receipt, preferably by e-mail, once correct payment has been received.

This option will also allow you to view and amend your details at a later time.

* There is the possibility for those in need to apply for financial travel support and for reduced registration fees, especially for young colleagues below 30 years of age. ESVCP members pay their fee through ESVCP see esvcp website. If ESVCP members wish to participate in Gala Dinner (17.09.2004) and excursion to Gdańsk (18.09.2004) they are kindly requested to contact: esvp@uwm.edu.pl 

STUDENT REGISTRATION:

A reduced registration fee is available for full-time students. For student registration, please attach a letter from the Dean of the Faculty certifying full-time student status.

 REGISTRATION FEE includes:

• Congress documentation.
• Attendance at Welcoming State Reception and Gala Dinner.
• Twice daily refreshments and lunch during full-day programme – Thursday and Friday and for ESVCP members also on Wednesday.
• Excursion to Gdansk (Saturday). Please confirm participation in this event in Accommmodation Form.

 ONE DAY DELEGATE FEE includes:

• Congress documentation.
• Attendance at Welcoming State Reception or Gala Dinner.
• Two refreshments and lunch.
• Excursion to Gdansk after € 50 extra fee (paid additionally to the one-day delegate’s participation fee).

 ACCOMPANYING PERSON’S FEE includes:

• Attendance at Welcoming State Reception and Gala Dinner.
• Sightseeing of Olsztyn and full day tour outside Olsztyn and two lunches.
• Excursion to Gdansk after € 50 extra fee (paid additionally to the accompanying person’s participation fee).

Please check carefully if Bank Draft was filled in correctly (Account Number should consist of 26 digits).
Please remember that given sums are the prices NETTO (do NOT include cost of the bank drafts) !!!

  e-mail (esvp@uwm.edu.pl) to the ESVP Secretariat.

 
CANCELLATION OF REGISTRATION – REFUNDS:

Before May 31^st 2004:                           80% of fee will be refunded.

Between June 1^st and July 31^st 2004      60% of fee will be refunded.

After August 1^st 2004                             No refund.

• Cancellation must be notified in writing to the Conference Secretariat in order to receive any reimbursement.

The point of acceptance of the CD with oral presentations will be run by the registration desk.
The presenters are required to leave their CD there.
